We participate in a variety of insurance plans. Many insurance plans require pre-authorization from your primary care physician. It is the patient’s responsibility to obtain such authorization prior to the appointment visit. Failure to obtain an authorization may result in rescheduling your appointment or denial of claims. Please bring a copy of your insurance card and stateissued identification at the time of your appointment. We file claims with all primary insurance providers and Medicaid insurance.

In the event we are not contracted with a specific plan or you are a self-pay patient, financial arrangements can be made with our Billing Manager.
